The Navigator Program provides 24/7 education, guidance and personalized support to families caring for loved ones with Alzheimer’s and other types of dementia. This program is grant funded and available at no cost to the residents of New Canaan. The Navigator Program is meticulously tailored to deliver free concierge level care management services to address the evolving and unforeseen needs that arise when supporting a loved one with dementia. This discussion will focus on an overview of dementia and caregiver needs, a review of the comprehensive services offered through the Navigator Program including our new Adult Day Program, identifying who benefits from and qualifies for the Navigator Program, and information on local caregiver support groups open to the community.
Kathleen Benevento is the Executive Director of the Navigation at Waveny LifeCare Network. She has more than 20 years of experience in various sectors of the healthcare industry including pharmaceuticals, durable medical equipment, homecare services and Telemedicine. For the past 5 years, Kathleen has been working directly with dementia clients and families to provide coordination of care and received training to become a certified dementia professional. Kathleen has successfully implemented and launched several patient and family support programs throughout her career and has a deep understanding of the importance of aging in place to improve quality of life. Other areas of expertise include managing business development and strategy, business process improvement, and client relationship management.
